How this website handles information, in plain language.

What this covers
This policy covers information collected through this website. Information you give us as a patient in the office is covered separately by our Notice of Privacy Practices, which describes how protected health information is handled under HIPAA.
What we collect
- Information you submit through the appointment request form: your name, phone number, email address, whether you are a new or returning patient, what you are booking for, your preferred time, and optionally your insurance plan and any notes you add.
- Standard server request data, including IP address and browser user agent, which is used for security and rate limiting.
- Anonymous usage measurement through Google Analytics, but only if you consent to analytics cookies. Nothing analytics-related loads before you choose.
What we deliberately do not collect
Our appointment form asks for scheduling information only. It does not ask for symptoms, diagnoses, medical history or any other health information, and we ask that you do not include such details in the notes field. This is deliberate: the form is not a secure clinical intake channel and we do not want it becoming one by accident.
We do not sell, rent or share your information with third parties for their marketing purposes. We do not add form submissions to a mailing list.
Email and the booking form
Appointment requests are transmitted by email through Resend, our email delivery provider. A notification goes to the practice and a confirmation goes to the address you provided.
Email is not a secure channel. Please do not send medical or health information to us by email or through the website form.
How long we keep things
Appointment request emails are retained for as long as needed to schedule and provide care, and then in accordance with our records retention practices. Analytics data is retained according to the Google Analytics retention setting configured on the property.
Your choices
- Withdraw or change cookie consent at any time through the cookie preferences control
- Ask us what information we hold about you from a website submission
- Ask us to delete a website submission, subject to any legal or clinical record-keeping obligations
- Use Global Privacy Control in your browser, which we honour automatically
Children
This website is not directed at children and we do not knowingly collect information from children through it. Appointments for children are requested by a parent or guardian.
Changes and contact
We update this policy when our practices change, and the date at the top reflects the last revision.

We schedule 2 patients an hour, so your exam is unhurried and your questions get answered. Most patients are seen within a few days.
