How we may use and disclose your information
- For treatment
- To provide, coordinate and manage your eye care, including sharing information with other providers involved in your care such as your primary care physician, an ophthalmic surgeon, or a retinal specialist.
- For payment
- To bill and collect payment from you, an insurance company or another third party, including verifying coverage and obtaining prior authorisation.
- For health care operations
- For activities necessary to run the practice, including quality assessment, staff training, and business planning.
- Appointment reminders
- To contact you about appointments, recall for annual examinations, and to tell you when eyewear or lenses are ready.
- As required by law
- Where federal, state or local law requires disclosure, including public health reporting and certain legal proceedings.
Your rights
- Inspect and obtain a copy of your health records, in most circumstances
- Request a correction to information you believe is incorrect or incomplete
- Request a restriction on certain uses and disclosures of your information
- Request confidential communications, for example asking us to contact you only at a specific number
- Receive an accounting of certain disclosures we have made
- Receive a paper copy of this notice, even if you agreed to receive it electronically
- Be notified if a breach occurs that compromises the privacy or security of your information
- File a complaint with us or with the Department of Health and Human Services, without any retaliation from us
Our responsibilities
We are required by law to maintain the privacy of your protected health information, to give you notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.
We will not use or share your information other than as described here unless you give us written permission. If you give permission, you may revoke it in writing at any time.

We schedule 2 patients an hour, so your exam is unhurried and your questions get answered. Most patients are seen within a few days.
