Keratoconus, scleral lenses and the conditions where the practitioner's experience determines the outcome more than the equipment does.

Specialty care covers the conditions a standard exam identifies but cannot treat, and where outcomes depend heavily on how often a practice performs the procedure.

Scleral lens fitting is the clearest example. It is an iterative craft, and a practice fitting two a year will not match one fitting them constantly. The same is true of orthokeratology and of managing a progressing keratoconus.
